At Berryfields Medical Centre on the  Buckingham Park Development in Aylesbury, the doctors and nurses take pride in offering the highest standard of patient-centred healthcare.  We run many clinics for chronic disease care and offer a wide variety of other medical services such as antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups.

Please do not come to the Surgery unless you have an appointment

If you are asked to come to the surgery you will be asked to wait outside until the clinician is ready to see you.  Please ring the bell or call us when you arrive and wait in the car if you have bought one.

Covid 19 Sick Notes.  We are sorry but this Practice will not issue a sick note for any absence related to Covid-19 however please find an agreed template for a sick note that you can download and populate with the appropriate details.Covid 19 Sick Note

Are you a Carer?  If so it is sensible you make an emergency plan to give details of the care you and others are providing in case you are taken unwell yourself.  Here is a link to an Emergency Plan you can complete.  Please forward any queries to Carers Bucks.

Registering a Death.  We have been advised that the process for registering a death has changed and should where possible be completed online - here is a message from the Registrar:

From Tuesday 31st March 2020 death registrations can only be made by families completing an on-line form on the Buckinghamshire Council website. This will be checked against the Medical Certificate of Cause of Death (MCCD) before registering.  For families without access to on-line facilities, there will be an option for them to call the registrar to provide the information.

For everyone’s safety and in line with Government direction we are minimising face to face consultations for the time being.  We will continue to work hard to meet your immediate care needs but to help manage this difficult situation and contain Coronavirus we will be:

Minimising face to face consultations.

Making use of online triaging services from Ask NHS and NHS 111 online – see below.

Minimising the amount of patients at the surgery at any one time.

Focus on urgent needs and restrict routine appointments.

Please note we are not able to carry out clinical checks for Coronavirus – only NHS 111 can direct this

Urgent Appointments

  • Help us to triage your needs by using the Ask NHS application before you call the surgery. You can download this application onto your phone using one of the links below.  This application will assess your symptoms and then offer you an appropriate course of action.  If required it will offer you a telephone consultation or tell you to call the practice.

  • We understand that not all patients (including children under 16) will be able to use an online application and will need to call the surgery in the normal manner. Please note there maybe delays in us answering your call and we apologise in advance for that.

Routine Appointments and Other Queries

We have temporarily disabled the booking of online appointments with Patient Access however if you have more routine needs please make full use of Patient Access and follow these steps:

Ongoing Medical Conditions.  It is anticipated that GP Practices will have to limit routine long term condition reviews.  The Practice will contact you if we are able to see you.  If you have any concerns or your condition worsens/becomes difficult to manage please call us or if you want advice e mail us on  

Repeat Prescriptions.  Use Patient Access.  We will be restricting the collection of prescriptions from the surgery and use electronic transfer to your nominated Pharmacy.  If you normally collect your prescriptions please e mail us on identifying your preferred Pharmacy.  Please do not come to the surgery to collect prescriptions unless we have called you.  If you are unable to use e mail or Patient Access please post your repeat request or put in the box outside the surgery.

Referalls.  We will send details of your referral by text and then in the post.  Please do not come in to collect referrals.  Please e mail us on if you need to update your e mail address.

Results.  You will be able to see your results using the Patient Access application.  If you have any concerns please e mail us on and we will get back to you.

New Registrations.  Please do not come to the Practice.  Instead please e mail us to let us know that you wish to register.  We will e mail you details on how to register.

Any other queries.  Where possible please e mail us on with your question and we will endeavour to get back to you as soon as possible.  If not then please do call – ideally after 10 am.

We do understand that it will not suit everyone to use applications and possibly e mail.  It does help us manage the situation but you can always call us.

2019 Patient Survey. Here is a link to the results of the 2019 survey of Patients - your comments really make a difference and we are grateful for the responses. The survey is conducted every year but do feel free to pass on your comments directly to the surgery

Call Recording.  For training and safety purposes we now record all calls made into and from the Practice.
